Knox County Community Health Center achieves NCQA PCMH status

Knox County Community Health Center has earned National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition.

The health center was recognized for meeting NCQA standards in areas such as team-based care, patient-centred access and continuity, care management and support, care coordination and transitions, and performance measurement. Project Director Anthony Bowersock highlights the center’s focus on long-term patient engagement and measurable improvements in health.

“Receiving this recognition reflects the dedication of our entire team to providing compassionate, coordinated care for every patient we serve,” Bowersock said. “We’re proud to offer a care model that focuses on building lasting relationships while helping our patients achieve better health outcomes.”

The PCMH model emphasizes placing patients at the center of their care by strengthening relationships between patients and their clinical teams. Research shows that PCMHs improve quality, patient experience, and staff satisfaction while reducing health care costs. 

The NCQA, a private nonprofit organization, is known for accrediting and certifying health care organizations while recognizing clinicians and practices in key performance areas. Its Healthcare Effectiveness Data and Information Set (HEDIS®) is the most widely used performance measurement tool in the industry, helping consumers and employers make informed health care choices.  

Knox County Community Health Center’s recognition places it among practices nationwide that demonstrate a strong commitment to quality improvement and patient-centered care. The achievement signals to the community that the center is dedicated to advancing health outcomes through structured, evidence-based practices.
