The Ohio District 5 Area Agency on Aging, Inc. is proud to announce vaccine clinics will be held from 9 a.m. to noon at the following locations:
Oct. 4 at Hawkins Conference Center, located at 2131 Park Avenue West, Ontario
Oct. 11 at Third Street Family Health Services, located at 600 W. Third Street, Mansfield
Oct. 18 at Ashland Health & Wellness Center, located at 309 Arthur Street, Ashland
Oct. 25 at Bucyrus Health & Wellness Center, located at 2458 Stetzer Road, Bucyrus
Anyone age 18 and older, and from any county, is welcome to participate in these vaccine clinics. Those interested in participating must register by calling the Third Street Clinic at 419-522-6191. COVID vaccines may vary so please confirm their availability at registration.
If transportation is needed, please call the Area Agency on Aging/Susan at 567-247-6458 and an appointment can be made. What to bring: A form of ID and your insurance card. You do not need to show proof of citizenship or residency status.
