Purchase of Service

The Ohio District 5 Area Agency on Aging, Inc. (AAA) is requesting Purchase of Service Proposals for 2023 from interested agencies and/or businesses to provide services under the Older Americans Act funds from the State of Ohio for persons aged sixty (60) years and older in Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca, and Wyandot Counties. These services are intended to assist clients to live independently as long as possible.

The Purchase of Service proposals for services funded are: Transportation, Homemaker, Legal Assistance, Evidence-Based Programs, Preventive Health Assessment, Congregate Meals Nutrition Program, and Home-Delivered Meals Nutrition Program.

The Proposal Application will be available beginning at 9:00 am on Wednesday, June 15, 2022 through the AAA website: aaa5ohio.org/ohio5.smapply.io

A Pre-bid Conference for all interested parties for Purchase of Service Proposals will be held on Tuesday, June 21, 2022 beginning promptly at 9:30 am via Microsoft Teams. Use the link on our website aaa5ohio.org, next to the application: or audio only option by calling: +1 323-673-4442,,721577640#. The purpose of the conference is to explain the process for submitting proposals, to answer questions and to offer technical assistance.

The application period will be open for proposal submissions from 9:00 am June 15, 2022 until 11:59 pm July 6, 2022. Proposals received after that time will not be accepted, and the application program will close.

Questions may be submitted prior to the Pre-bid Conference by emailing Kim Mercer, Quality and Planning Manager at kmercer@aaa5ohio.org or by calling 567-247-3479. All questions will be answered/discussed at the Pre-bid Conference on June 21, 2022.
